Once you have entered our homepage, a textbox will pop up, asking if you’d like to schedule a virtual appointment. If you would, click into the box, and you will be prompted to fill out a form. The request form will have you put in information such as your:

- Preferred date and time for an appointment.
Once this is filled out, you’ll have to wait for a member of our office to confirm you have been scheduled. Then, at the time of your appointment, return to this request form, scroll to the bottom, and click the check-in button to enter the secure video platform.
